Medical matters and official complaints cannot be dealt with via this form.

You can also access our feedback questionnaire (Friends and Family Test FFT) below where you can leave any comments or compliments.

We make every effort to give the best service possible to everyone who attends our practice. However, we are aware that things can go wrong resulting in a patient feeling that they have a genuine cause for complaint. 

To pursue a complaint please complete the complaints form and the practice manager will deal with your concerns appropriately.  Alternatively you can put your concerns in writing to the practice Manager Joanne McElwee.  This can then be sent to us by email at stokesley.mailbox@nhs.net , by post or handed in at the desk.
