Accessing someone else’s information (Proxy access)

As a parent, family member or carer, you may be able to access services for someone else. We call this having proxy access. We can set this up for you if you are both registered with us.

Healthy You Weight Management Service

Healthy You is designed to support you or you and your family to help you meet your lifestyle goals, which might be to be more active, eat better, or manage your weight.

You are eligible to access Healthy You if you are actively thinking about making lifestyle changes and are either:

  • an adult with a body mass index (BMI) equal to or greater than 30 (or equal or greater than 25 if you are from a black or minority ethnic group or have a condition such as Type 2 diabetes, heart disease or hypertension) 
  • a child aged four or above at or above the ninety first centile for BMI - exceptions will be made for children and young people with no centile or BMI measure available

Accessible Information

Easyhealth is a website where you can find ‘accessible’ health information. ‘Accessible’ information is information that uses easy words with pictures. There are over 500 leaflets on Easyhealth, made by many different organisations. You can print off nearly all the leaflets straight away.
